Major | State of injury (i.e., disability/impairment) that is permanent OR requires major medical intervention. Examples: birth defects, disfigurement, broken bones, organ damage. | 4 |
Moderate | State of injury (i.e., disability/impairment) that is NOT permanent BUT may require minor medical intervention. Examples: symptomatic dizziness, infection requiring antibiotics. | 3 |
Minor | Temporary AND non-debilitating state of pain/discomfort. Examples, upset stomach, headache, minor puncture wound (e.g., accidental sterile needle stick). | 2 |
